The only time I dealt with something similar was during a period I was
uninsured...but was still going through some medical treatments.
I told both the Dr office and the physical therapy office I didn't have insurance. They told me what it would cost, threw me a 50% or more discount if I paid cash before leaving the office, and it was mostly good.
In one case at the Dr's office, I was told the office visit would be $75. Fine. While there, they gave me a steroid injection. Also fine. But then they sent me a bill for $500 for the injection. I argued with them that I didn't have insurance, I was told it would be $75, and any additional charges should have been cleared with me. In the end, they just waived the $500 entirely.
But, I was asking what the cash price for services was, not what the out-of-pocket-after-insurance price would be.
