Much of the country confuses
health insurance with health care.
You buy collision insurance for your car in case something really bad happens. The cost of oil, tires and occasional tune up is not covered by insurance - it is a cost to care for your car and there are consequences if you don't care for it.
Health insurance is for big ticket and emergency type items, health care needs to cost people so everyone has some skin in the game. Make it a sliding scale based on income or something similar.
Someone with a higher income and poor financial skills (need to plan for health insurance AND health care costs in addition to new cars, cell phones and big screen TVs) might get left out of some procedure - but that's going to happen to someone with ANY system - and it is not going to be "fair". I'd just as soon it be people with enough smarts to make some money, but dumb enough not to plan. I also know there will be exceptions where this is terribly unfair - just like now and again will be with ANY system.
Might take a generation or so, but people would learn.
