My BIL suffers from mental illness. He threatens suicide when he goes off his medication. His sisters...who are very involved in his care...are good about stepping in at those moments to see what the deal is. Every time...it's because he has stopped taking his medication. The sisters manage to right the ship...and off he goes. But yes...it's terrifying...and the family has learned to live with the fear that he may make good on the threat some day.
