It is also wise that in identifying possible solutions you identify what the true long term objectives are and the most cost effective way to get there. In some cases you blow it up entirely while in others you manage and support differently to obtain desired behaviors from your subordinates. I think this situation has gone from bad to worse because problems/root causes were not properly and timely identified and addressed. They need to get it right going forward before a treatable wound becomes a fatal injury.
