I really don't think that's very common at this point
certainly not for the grad student to get no credit for their work. Sure, a dissertation committee chair/member might list themselves first on a publication when it seems like the grad student did most of the work. Mine didn't, but I really wouldn't have cared if they did. I did most of the work, but their guidance throughout and specifically their knowledge of how to navigate the publication process was invaluable.
As a Master's student, I had a professor ask me if he could turn a paper I wrote for a class into a journal submission. His name ended up first on it, but I was there too...and while it was fundamentally my work, he added a fair amount of content and handled the publication process 100% on his own.
