Hard to find data, but here's what I found. Over 95% hospitalized...

Of the 323 confirmed cases in those ages 12-29, 95.7% of the cases involved hospitalization, but no reported deaths. I have seen other newspaper accounts of deaths from myocarditis that family members expected were due to the vaccines (one was in Israel), but obviously these were not included in this report.

"In a look at outcomes of 323 confirmed myocarditis or pericarditis cases in people ages 29 and younger, 309 were hospitalized, but 295 were discharged. Of those 295, 218 were known to have recovered. Only nine were known to still be in the hospital, with two in the intensive care unit. Fourteen of the 323 were not hospitalized."
