I think the main reason "going on a diet" doesn't work is that eventually


you go off the diet.

I lost about 50# myself a little over a year ago and have kept it off so far. Part of the key for me was I never really went on a diet.

As you say, read labels, make informed choices (bacon is AWESOME!), track what you eat, watch portion sizes. If you eliminate anything you love entirely, you will probably fail. Try cutting back instead.

It can be done.
