That assumes perfect vision..
Sometimes you can have an employee who absolutely is working out, has the right skillset and succeeds for a time. For any number of variable reasons something can change thier performance and make it no longer acceptable. I dont see how that's a failure of managment. Failure would be not seeing or refusing to acknowledge that something has changed.
