I would grade the MPG drop being worth it to avoid those engine issues down the line. Longevity and reliability are at the top of my "wish list." One day GM may solve the lifter issue...maybe switch back to mechanical lifters? (I know that isn't going to happen...lol)

If you plan to keep this truck "long term," recommend going with a stainless steel aftermarket exhaust. I would also look at installation videos to see which ones you can install yourself. Some require welding, and others will bolt in. The bolt in "cat back" systems are fairly straight forward and easy to install yourself. Just make sure you order the correct one for your truck. They will offer different lengths for standard cabs, extended cabs, and crew cabs.
