I think the answer to the question is a matter of overhead, even after you factor in shipping. Overall in store pharmacies make money but not that much. They serve mainly to attract customers for other goods. That's why you see so many dedicated pharmacies failing, getting absorbed, etc. We have had a few professional acquaintances and friends try to hang their own shingle and they have tried hard to innovate. The money just isn't there
