I'm so sorry to learn of your loss

and dealing with those types of issues just seem so inconsequential under these circumstances. However , I'd think at the very least you will/could get access to EOB sheets wrt those insurance claims , esp. since he was under you coverage. As far as the taxes go you may be able to get answers from the IRS website about those specific circumstances or by giving them a call. I'd contact the employer wrt the IRA issue , who may then refer you to the plan admin. Take care.
