Depending on what I see on their resume, I always ask:
If it's applicable:
"I see a 3-month gap in work history here. What did you do for those three months."
Look them dead in the eye when you ask this and watch to see which direction their eyes dart (down and away indicates subterfuge - up and to the left indicates formulating a response that may or may not be truthful).
About 2/3 through the interview when you think you've got them relaxed, drop your professional face and try to come across as more of a confidant "you can tell me anything", then say, "tell me about your current manager". If they trash their boss, that's a problem (especially if the criticism is extreme). If the report is glowing, ask why they are leaving and if they have confided in their supervisor about their job search.
The answers to those questions can sometimes be pretty helpful.
