We had about 25 people around my mom when she died, and my dad was holding her hand the entire time telling her that it was okay to let go, that it was okay to die and he'd see her later. The doctors had said it could take up to three hours after we removed her from the breathing valve for her to die; it took ten minutes.

I also think a lot of people know when it's their time as well. My mom had ALS and was in and out of the ICU for close to two years. Each time she asked if she was going to die, and we said no. Except the last time -- she never asked us. She knew it was her time and made sure we did too.
